What to Bring
To ensure a smooth check-in process and a comprehensive clinical evaluation, please have the following items ready for your appointment.
Medical Reports
X-rays, MRIs, CT scans, and relevant clinical info.
Medication List
A current list of all prescriptions and dosages.
Picture ID
A valid government-issued photo identification card.
Insurance Info
Primary and secondary insurance cards.

If you need to reschedule, please telephone the office during business hours and allow at least 1 days’ notice. This courtesy allows us to offer your appointment time to patients on our waiting list.
